Community Pharmacy PGDs

From 20 May 2024, we will no longer require a paper or emailed copy of a signed PGD from your Pharmacy. Please keep these for your own records.

Pharmacists do not need to submit their various signed PGDs to Pharmacy Admin Team. Instead please use the link below to submit PGD information to NHS Borders:

The information gathered from this form will allow NHS Borders to fulfil its responsibilities to keep a list of all pharmacists operating in its area who have signed the PGDs currently operating within the Board.

Some PGDs are slightly out of date - these can be used while we await a National update.
