How to refer to the Wellbeing Service
We accept self-referrals from members of the public. Please complete a self referral form and email it to wellbeing@borders.scot.nhs.uk or call our admin team on 01896 824502 to make an apppointment.
GPs and healthcare professionals can refer patients via SCI Gateway or by using a healthcare professional referral form and sending it to wellbeing@borders.scot.nhs.uk
Clinicians using Trak and Badgernet can make referrals using a single referral form within these systems.
If you work within the community, people can self refer or you can also complete the communities referral form on their behalf.
Please note referrals for weight loss will be passed to the Borders Weight Management Team.
