This is a healthy lifestyle service for children, young people (aged 0-18 years) and their family.
Our programme offers practical tips to support you to eat well and be active.
Our experienced team can support you to make positive lifestyle changes.
We will work with you to identify specific goals that you would like to achieve, to give you the best support.
If you would like to make a self-referral to our service, please complete this Self-Referral Form and return it to us via email or post. If you would like help filling out the form or wish for us to do it for you, please contact us.
If you prefer you can speak to your GP, Health Visitor, School Nurse or another health professional.

What to expect:
Once we have received your referral form you will be contacted by our team who will arrange your first session with our Dietitian.
Young people can be seen individually or as a family or with a parent/guardian depending on their circumstances.
These sessions may be face to face or virtual by video conference (Near me).
