Information for patients about to leave hospital
When you leave hospital make sure you take all your belongings with you.
If you brought your pills or medication with you, make sure you ask your nurse for them to be returned to you. However, it may not be appropriate for your medication to be returned to you if, for example, your medication has been changed during your stay in hospital. Your doctor, nurse or pharmacist will tell you about this.
If you are on any medication, we will give you a supply to take home and you should get further prescriptions from your own doctor.
The hospital doctor will write a discharge letter for you to take to your doctor who will receive a more detailed letter within 10 days of you going home. Your nurse will give you advice on getting back to normal activities. He or she will discuss any restrictions with you. If you need a district nurse, your nurse will give you a letter explaining your future treatment needs.
